Our services do not replace comprehensive primary medical care or emergency services. If you are experiencing a medical emergency, you must dial 911 or visit the nearest emergency facility immediately.
Individual Biological Responses
All clinical medical results—including outcomes from weight loss injections, hormone replacement therapies, and lifestyle transformation adjustments—vary fundamentally by individual. Transform Clinix does not guarantee explicit timelines or specific aesthetic or physical baselines. Your treatment plans are built dynamically on your distinct blood work, biometric markers, and physical health history.
2. Telehealth & Medical Diagnostics Requirements
Mandatory Baseline Blood Work
To qualify for any advanced prescription protocols (such as GLP-1 medications, TRT, or clinical injectables), you must submit to comprehensive laboratory diagnostics coordinated by our medical staff. Transform Clinix reserves the right to decline or alter treatment plans if baseline diagnostic panels reveal underlying health risks or contradictions.
Verification of Patient Information
By utilizing our in-clinic or telehealth services, you certify that all health histories, symptoms, and lifestyle data provided to our clinical team are completely truthful, accurate, and up to date. Falsifying biological data to obtain prescription therapies is strictly prohibited and constitutes a violation of this agreement.
3. Prescription Medications and Pharmacy Fulfillment
Accredited Compounding Pharmacy Sourcing
Transform Clinix partners exclusively with state-licensed, PCAB-certified compounding pharmacies to supply premium wellness prescriptions. Medications are formulated under strict safety, sterile conditions, and regulatory guidelines.
Use and Administration Rules
Prescribed treatments, including weight loss injections and peptide therapy kits, are meant solely for the individual named on the medical prescription label. You agree not to distribute, sell, share, or alter any therapeutic medications provided to you by our clinic. Self-administration must follow the explicit guidance provided during your medical coaching consultations.
4. Financial Terms, Program Fees, and Cancellations
Payment Obligations
By enrolling in our medical programs or scheduling an advanced wellness evaluation, you agree to the specified financial fees for lab panels, clinical consults, and recurring medication shipments. All processing transactions must be completed prior to lab routing or pharmacy fulfillment.
Refund and Subscription Modifications
Due to the custom medical compounding and personalized configuration of laboratory metrics, prescription orders are non-refundable once submitted to our pharmaceutical partners. Cancellation of subscription lifestyle transformation programs must be submitted to our administrative staff at least five business days before your next designated processing schedule.
5. Intellectual Property & Acceptable Use
All digital platforms, text architectures, high-converting copy models, localized content pages, images, and branding assets displayed across our websites are the explicit intellectual property of Transform Clinix. Users are prohibited from copying, distributing, or scraping page contents for competitive commercial usage without our direct, written consent.
6. Limitation of Liability
Transform Clinix, alongside its clinical staff, licensing directors, and operational partners, shall not be held liable for any indirect, incidental, or secondary wellness complications arising from a patient’s failure to adhere strictly to the safe self-administration instructions, dietary adjustments, and tracking guidelines established by our healthcare providers.
7. Revisions and Local Jurisdiction
Transform Clinix reserves the right to modify these Terms and Conditions at any time to align with shifting federal telehealth mandates, state health laws, and corporate policies. Any legal actions or resolutions concerning our clinical operations will be governed under the state laws of Utah.
